Position Overview:
Successful candidate will have HL experience and/or solid Home Health experience.
This position, in cooperation with Primary Care Physician, hospitalist and hospital staff, facilitates the Hospice admission process. This position is responsible for attending hospital rounds, facilitating orders, assessing patient needs, including level of care, Durable Medical Equipment (DME), comfort medications and transportation. This position enters referral into electronic medical record and completes referral process and admits patient as appropriate. This position forwards hospital documents to hospice and communicates with hospice and hospital staff. This position provides education to hospital staff and physicians. This position is able to make home hospice visits to provide patient care coverage as the business need requires.

EDUCATION:
- Graduate from an accredited licensed vocational nursing school
CERTIFICATION & LICENSURE:
- LVN-Licensed Vocational Nurse
- BLS-Basic Life Support Healthcare
- DL-Valid Drivers License Class C
- AUTO-AutomobileInsurance
TYPICAL EXPERIENCE:
- 5 years recent relevant experience
SKILLS AND KNOWLEDGE:
- Understands the diagnostic criteria for GIP eligibility.
- Must have clinical knowledge of disease entities common to hospice/home health.
- Commitment to teamwork and customer service, both internally and externally
- Collaborative and problem-solving skills with customers and management. Organizational skills.
- Must have telephone, interpersonal and written communication skills.

Pay Range is $38.87 to $50.53 / hour
The salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled.
