Essential Duties:
Under the general supervision of the Care Coordinator Supervisor, the Care Coordinator is responsible is a key representative for cultivating quality relationships inside multiple medical facilities, being visible on a daily basis providing prompt service, informing patients and family members regarding delivery and possible co-pays, train patient on how to use the equipment delivered, request medical documents when needed, while marketing a variety of SuperCare Health DME, Respiratory therapies and/or infusion services.
Responsibilities:
- Coordinating all details of Durable Medical Equipment and Respiratory item deliveries to the Hospital room and to home with the Customer Care Specialist, Dispatcher and Respiratory Department.
- Consulting with the patient and and/or family on receiving the Durable Medical Equipment items to the patients’ room prior to discharge whenever possible, to reduce the amount of deliveries to home.
- Instructing patient/family and caregivers on the appropriate use of Durable Medical Equipment and Respiratory equipment when delivered to hospital room.
- Consulting Care Coordinator Manager on any Durable Medical Equipment Respiratory orders for patients.
- Monitor and report referrals from partnered healthcare systems vendors to other companies other than SuperCare Health.
- Ensure partnered vendors and patients satisfaction by completing all procedural and requested follow through.
- Maintain a professional and constant interfacing with patients Managed Care and Primary Medical groups to ensure all required pre-authorizations and proper coordination of services when applicable.
- Complete and submit on time all required daily and weekly reports re: activities, patient and vendor interaction.
- Must review and maintain appropriate inventories of equipment in assigned storage/closets to maintain adequate inventory PAR levels set by SuperCare Health.
- Provide Customer Care Department Specialists with essential information needed to properly process insurance or private pay billing.
- Adhere and abide to SuperCare Health’s HIPPA policy.
- Understand and adhere to all of SuperCare Health company policies.
- Must submit completed all approved business expenses timely manner.
- Must attend all department meetings and in-services scheduled.
- Schedule ongoing community development activities, in-services and seminars on any new Durable Medical Equipment.
- Perform all other duties as assigned and required by Department Leadership.
Physical Demands:
- Must be able to walk long distances across large healthcare facilities.
- Must be able to lift and carry equipment and miscellaneous items up to 75 lbs. on a daily basis.
Work Environment:
Fast-paced Healthcare Facilities with the emphasis on prompt deliveries and coordination of Durable Medical Equipment and services with an effectively with a high level of communication with multiple partnered healthcare vendors. Demands of this job include, but are not limited to coordinating multiple discharges at one time, working under multiple deadlines, and forwarding pertinent discharge information to both SuperCare Medical and appropriate Healthcare staff.
Work Experience Requirements:
- Experience with Medical Supply Company
- Knowledge of diagnosis and Medical terminology
- Excellent Customer Service skills
- Experience in healthcare, medical and/or HME industry (preferred).
- Be able to work on multiple tasks and plan and prioritize actives to achieve results and meet deadlines.
- Strong organizational skills and detail oriented.
