Job Abstract

The Medical Care Specialists will work closely with program manager, medical providers and other program staff to facilitate recommended treatment plans, to verify that services are delivered according to the clients’ needs concerning medical needs, health literacy and cultural competency.

The Medical Care Specialist will ensure that appropriate medical care is provided to HIV positive clients at the site. The Medical Care Specialists will plan and coordinate health care services appropriate to achieve the goal of medical rehabilitation of HIV positive clients. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integration of services. The Medical Care Specialists will maintain detailed and accurate records, including encounter documentation, and will keep the client's primary care provider informed of the client's progress in the adherence program.

1. Compliance with Employee Health Services.
2. Complete intake and initial and annual psychosocial assessment.
3. Work closely with Program Manager to ensure that Transgender Family Program services are offered according to program deliverables.
4. Provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient need.
5. Develop, implement and coordinate a medical care plan with the client's health care providers, as well as the clients and his/her family and evaluation of treatment results.
6. Coordinate the integration of social services/case management functions into the patient care, discharge, and plan processes with other departments, external service organizations, agencies and healthcare facilities.
7. Mobilizes resources, as needed, to achieve expected goal to assist in achieving desired clinical outcomes within the desired timeframe
8. Provide HIV prevention and harm reduction education to HIV positive clients in both one-to-one and group format.
9. Act as patient advocate: investigates and reports adverse occurrences, and perform staff education related to resource utilization and psychosocial aspects of healthcare delivery.
10. Provide coaching, appointment assistance, treatment adherence counseling and education, regular plan update and re-evaluation.
11. Facilitates interdisciplinary patient care rounds and/or conferences to review treatment goals, optimize resource utilization, provide family education and identified post-hospital needs
12. Maintain detailed records of client access to services and update clients’ charts regularly.
13. Participate in Quality Improvement activities and monitor Quality Improvement indicators as directed by supervisor.
14. Participate in on-site and off-site program activities.
15. Adhere to departmental goals, objectives, standards of performance, and policies and procedures.
16. Performs other related as assigned by supervisor.

Certificate/License required:

Associate degree in healthcare related field preferred. Bachelor’s degree is preferred. Experience working with HIV positive patients and current knowledge of the latest HIV treatment advances.

OTHER FUNCTIONS:

1. Promote effective and efficient utilization of clinical resources.
2. Assist staff in implementing and facilitating weekly workshops, support groups, outreach activities and program initiatives.
3. Participate in weekly staff meeting and individual supervision meeting.
4. Participates in training, in-services, and support activities.
5. Participate in HIV Team meetings as appropriate.
6. Distributes HIV/AIDS education materials and condoms to clients.
7. Participate in on-site and off-site program activities.
8. Performs other related duties as assigned by supervisor.
