The Social Worker is responsible for completing Intakes, provide individual group and family therapy, psychoeducation services as well as referrals needed for ongoing mental health services and community needs. Essential Functions:

Performs initial diagnostic evaluations
Prepares and maintains statistical reports and records as required by regulatory, organizational and official agencies
Provides individual, family and group therapy
Formulates treatment plans
Provides psychotherapy, assessment and evaluation services
Performs other duties as designated by the department
