Summary:
The Care Management Assistant (CMA) provides administrative support to the licensed members of the Care Management team through clerical assistance with care coordination and transition planning efforts. The CMA supports efficient movement of patients to post-acute facilities including Skilled Nursing Facilities (SNF), Long-term Acute Care Hospitals (LTACH), Inpatient Rehab Facilities (IRF) and Intermediate Care Facilities (ICF) by receiving task triggers to create referrals; transmit to post-acute care provider facilities via HIPAA secure platform and triggering facility pre-certification initiation, or initiates pre-certification as directed.
The CMA supports department compliance to federal and state regulations by serving patient rights notifications related to Discharge Planning (Important Message from Medicare) and Utilization Management (Medicare Outpatient Observation Notice). The CMA works cooperatively with internal multidisciplinary teams, providers, allied health staff, as well as patients, caregivers and visitors. At the direction of the licensed staff (RN TCC or SW), the CMA assists with arranging transportation to facilities or arranging follow-ups.
The CMA works collaboratively and effectively with external community providers/agencies, facility admissions staff and insurance companies. The CMA position requires the individual to take initiative, be proficient, and have strong decision-making skills, organizational skills, communication skills and a commitment to quality and excellence in service.
Formal Education Required: a. High school diploma or equivalent
Experience & Training Required: a. Minimum of one (1) year experience in an administrative or business setting that requires supporting colleagues and working with data b. Basic experience using a computer is required c. Previous experience in healthcare preferred d. Knowledge of medical terminology preferred e. Minimum one (1) year experience working with electronic health record (EHR) preferred f. Previous insurance and prior authorization experience preferred

Ability to complete multiple tasks simultaneously d. Maintains confidentiality of all patient information e. Demonstrates basic computer skills as well as navigation and application of electronic devices and platforms f. Knowledge of CPT and ICD-10 codes g. Requires interpersonal skills sufficient to interact effectively with patients, their families, interdisciplinary caregivers, peers, subordinates, and supervisors. h. Excellent verbal and written communication skills i. Population Specific Competency: Ability to effectively interact with patients/customers with the understanding of their needs for self-respect and dignity

$15.91/hr - $19.09/hr
The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay. We believe in the importance of pay equity and consider internal equity of our current team members when determining offers.
