Position Overview:
Under the supervision of the Director Patient Logistics, provides general oversight of patient placement and flow throughout Capital Health (CH) through the utilization of the bed tracking system on a continuous basis. Facilitates processes on a daily basis to ensure patient care quality is expedited and patient satisfaction is maintained at the highest level. In collaboration with the multi-disciplinary team, coordinates, prioritizes and optimizes patient flow activities throughout CH, as well as patients transferred in from other hospitals and through direct admissions. Ensures that overall effective transfer of the patient is completed in an efficient and timely manner. Supports and enhances the Administrative Coordinator, Nurse Manager, Department Managers, RN and Case Manager who are directing and managing safe patient care and transfer of the patient. Facilitates coordination of transfers among inpatient units. Runs various logistic reports on a monthly basis.
MINIMUM REQUIREMENTS
Education:Associate's degree and graduate from accredited school of nursing.
Experience:Four years experience in an acute care hospital setting to include minimum Telemetry experience. Critical Care experience preferred.
Other Credentials:AHA BLS - Healthcare Provider,Registered Nurse - NJ
Knowledge and Skills:Strong verbal and written communication skills. Excellent customer service orientation skills necessary to communicate effectively and utilize effective conflict resolution and negotiation skills to support a cohesive multidisciplinary team and ensure optimal patient outcomes. Possesses ability to use critical thinking, logic and reasoning to address complex situations. Demonstrates responsiveness to needs of patients, other care team members, and self. Ability to organize and prioritize, in a fast-paced environment. Ability to leverage interdepartmental relationships to remove obstacles.
Special Training:Possesses computer skills that include data input into multiple systems, working knowledge of windows based applications such as Microsoft Office skills, and ability to learn hospital based programs for access to lab results, bedtracking, etc. Able to abstract medical information.
ESSENTIAL FUNCTIONS
- Coordinates the overall functions of the Logistics Center in conjunction with the Director. Coordinates bed flow functions through the management of the electronic bed tracking board.
- Provides daily operational oversight and decision making of patient flow activities throughout Capital Health by collaborating with Patient Access, Emergency and Surgical Services departments, as well as physician offices, in assessing, planning and implementing patient flow activities to promote patient and customer satisfaction.
- Communicates with clinical unit staff the new admission information and develops a plan for acceptance of the new admission within thirty (30) minutes.
- Collaborates with physician, Case manager, and BedTracking to ensure timely coordination of discharge and transfer. Communicates and collaborates with physicians regarding transfer/discharge orders. Assists primary RN, as needed, with outstanding issues related to patient transfer/discharge.
- Assesses patient status/acuity to facilitate the efficient movement of patients through the hospital system by prioritizing inpatient admissions, critical care admissions, and internal/external transfers.
- Serves as an expert clinical resource for patient placement by ensuring smooth functioning of the Logistics Center process to reduce wait times and avoid unnecessary movements of patients. Serves as contact person for bed management as it relates to admissions.
- Ensures throughput on units and compliance with transfer process through management of people and process. Delegates tasks appropriately and employs resources as needed to ensure high quality and efficient care related to patient flow.
- Identifies and anticipates barriers to discharge/patient flow activities and works to ensure throughput in ED, inpatient units and compliance with transfer process through management of people, metrics and process improvement.
- Activates the Hospital Surge/Capacity plan based on the guidelines established and works to initiate/activate/manage/close identified overflow patient care areas.
- Manages patient flow using data (Logistic Dashboard), rounding and daily metrics.
- Reviews blocked and flagged beds daily to complete daily closed bed report with bed numbers and reasons for closure. Communicates discharge times and transfer unit information in bed tracking system when appropriate.
- Monitors inpatient bed status and upgrades bed cleaning status via bed tracking system to expedite patient flow to a clean bed.
- Coordinates with Transport and Environmental Services supervisors to expedite patient flow.
- Communicates openly and effectively by providing ongoing education to the health care team on the overall patient flow strategy on rationale for patient placement.
- Uses effective and appropriate communication styles with patients, significant others, visitors, staff and professional colleagues. Promotes team momentum, enthusiasm and pride.
- Functions as a department team member to support unit and Capital Health?s goals, objectives and established policies.
- Performs other duties as assigned.
