Summary:
Responsible for collaborating with Summa Health Medical Group, clinical and operational areas to identify, create and implement care delivery processes that will promote optimal outcomes in Medical Group quality initiatives. Responsible for supporting organizational performance improvement including compliance to regulatory compliance, third party payer targets and Advanced Payment and Quality Models through interfacing with members of medical staff, department managers and directors and ACO. Leads the development and implementation of processes to track key metrics to assess health outcomes, service excellence, and efficiency of care provided to patients consistent with Medical Group and System objectives. Responsible for the management of Clinical Staff and Non-Clinical roles that provide coordination and/or clinical support to the population health strategy in the Medical Group.
Formal Education Required:
a. Bachelor’s degree in nursing or RN with Bachelor’s degree in health related field
b. Ohio Nursing License required
Experience and Training Required:
a. Three (3) years experience working in a clinical practice setting, preferably ambulatory or office-based setting
Other Skills, Competencies and Qualifications:
a. Experience with population health, NCQA, HEDIS and CMS standards/requirements
b. Experience with data analysis, metric and trending methodology and benchmarking in the quality improvement and outcome areas
c. Experience and demonstrated skill set in management of team members focused in Population Health Strategies
d. Epic EMR experience preferred
e. Population Specific Competency: Ability to effectively interact with patients/customers with the understanding of their needs for self-respect and dignity

$37.03/hr - $55.55/hr
The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials.
