The Social Services Coordinator is key in Active Day’s success! As Marlton’s Social Services Coordinator, you will be responsible for planning, organizing, developing, and delivering all essential social services to meet the needs of our members and their families/caregivers. As Social Services Coordinator, you will be responsible for all Social Service Assessments and Summaries and provide case management services to address the psychosocial and socio-economic needs of members and their families or caregivers. You will communicate regularly with the Center Director and clinical team to ensure members receive the most comprehensive and quality of care possible. You will also ensures all care is conducted within HIPAA regulations and per all state-required laws and regulations.

Evaluates members referred to the Center to determine appropriateness for admission. Assists with referrals to other agencies when a member is not appropriate or becomes inappropriate to participate as a member at the Center.
May explore and evaluate financial situations and assist members in using appropriate resources.
Participates in community outreach programs, including communication with other health and social service providers and inquiries from prospective members. Participates in Center marketing
Provides active assistance with members as needed (ie: hygiene, meals, ambulation, arrivals and departures, and social activities).
Qualifications
Qualifications
CSW with B.S. degree in Social Work or MSW degree
Minimum one (1) year experience in a health care facility, preferably working with seniors and/or adults with disabilities.
Knowledge of applicable state and federal environmental and OSHA regulations.
Knowledge of the state's Department of Health and Senior Services Manuals of Standards for Adult Day Health CVare Licensure very helpful and therefore preferred.
Extremely strong collaborate skills across all levels of staff and thrives in a collaborative and team environment.
