A new opportunity has become available for a Director of Case Management at a 500+ bed highly accredited facility in central California. This is a very large role that is in need of someone that will be dedicated to the department as well as the hospital's goals and values.

The Director of Case Management plays a crucial role in overseeing and managing the case management department within a large hospital setting. This position is responsible for ensuring efficient utilization of healthcare resources, facilitating patient care coordination, and managing the discharge planning process. The Director of Case Management collaborates with various healthcare professionals to optimize patient outcomes and ensure compliance with regulatory requirements and hospital policies.

The Director of Case Management plays a pivotal role in ensuring the delivery of high-quality, patient-centered care while optimizing resource utilization and operational efficiency within the hospital setting. This position requires strong leadership skills, clinical expertise, and a commitment to continuous improvement in healthcare delivery.

There are a few requirements in order to be considered for this opportunity which may include, but are not limited to:

• A bachelors degree in related field
• Previous management experience over care management
• Current licensure or certification as required by state regulations (e.g., Registered Nurse (RN), Licensed Clinical Social Worker (LCSW) preferred
