Position Objective:
To provide linkage to continuous and comprehensive services necessary for the promotion of optimal mental health and the prevention and treatment of psychiatric and substance use disorders to patients across the age spectrum. The clinician works collaboratively with medical and mental health providers, as well as community agencies and primary care providers. Clinician efficiently and effectively discusses patient needs with patient, provider and family via phone, and refers appropriately to treatment, ensuring the patient is referred to the appropriate level of care.
Essential Job Duties:

- Provides consultation and education to providers in the implementation and coordination of multi-disciplinary care.
- Provides referrals to the patient for outpatient mental health treatment and substance abuse treatment.
- Serves as a liaison between the patient and ancillary services, e.g., treatment centers or programs as well as serving as the direct contact to the patient.
- Refers patients to other resources or services in the community, such as support groups and recovery groups.?
- Maintains a customer-oriented approach to meeting patient needs in an expedient manner, involving the family in the plan of care as appropriate.
- Interviews and assesses patient using behavioral health knowledge and skills in making referrals.
- Recognizes process needs and facilitates appropriate change through collaboration with Behavioral Health Leadership.
- Develops and maintains detailed list of community resources for referral.
- Collaborates with Community Behavioral Health Navigator regarding community resources and accessibility of services.
Educational/Experience Requirements:
- Masters Degree in Counseling or Social Work required (Social Work preferred)
- Two years of experience in the field of behavioral health.
