Job Abstract

1. PURPOSE OF JOB:

Analyzes medical records for missing and/or incomplete clinical information, including physician reports, dates and signatures to comply with Joint Commission, CMS, PA Dept of Health, other external agencies and Doylestown Hospital's Medical Staff Bylaws. Scans and assigns the accurate form identification to medical record forms/documents into Meditech according to Department Policy and Procedure.

2. ESSENTIAL FUNCTIONS:

1. Preps, scans, and assigns the correct form identification to records according to the hospital's standard chart order for analysis of complete clinical documentation. Reviews each record for complete physician documentation, including signatures, on both the paper and electronic medical record as required by CMS, Joint Commission, PA Department of Health, other external agencies and DH Medical Staff Bylaws.

2. Scans and assigns form identification of documents according to Department procedures maintaining a 98% accuracy rate.

3. Performs quality analysis of scanned documents demonstrating a 98% accuracy rate.

4. Assigns chart deficiencies utilizing Meditech's Incomplete Records Desktop and Analyst Desktop module.

5. Files birth certificates with the Pennsylvania Department of Vital Records.

6. Demonstrates a working knowledge of Department equipment.

7. Performs other HIS-related duties as required.

QUALIFICATIONS:

1. Education: High school diploma or equivalent.

2. Experience: Clerical skills; hospital/medical office experience preferred.

3. Other Skills: Computer and Data Entry skills, Medical terminology, attention to detail and ability to multi task required.
