We are seeking a Community Care LVN to join our team, providing essential health services and support to our family members through home visits and telephonic outreach. This role is accountable for conducting basic health assessments, offering health information, and making appropriate referrals, all while embodying our core values of trust, empathy, commitment, humility, creativity, and community-mindedness.

Key Responsibilities:

• Establish trusting relationships with family members and provide general support.
• Conduct physical exams and assist with remote clinician-supervised exams.
• Perform ongoing follow-ups, motivational interviewing, and goal setting.
• Manage clinical supplies, facilitate telehealth visits, and complete required forms and screening tools.
• Coordinate care with medical providers and other clinical personnel.
• Maintain accurate records in the EMR and ensure HIPAA compliance.

Qualifications:

• Licensed Vocational Nurse from an accredited US facility required.
• At least one year in a health services environment; community health experience preferred.
• Proficiency in Microsoft Office applications.
• Bilingual in Spanish and English preferred.
• Strong communication skills and ability to manage relationships effectively.
• Reliable transportation for regular travel to family members' homes and other locations.
