As the Administrative Director of Care Management, you will oversee the daily operations and strategic direction of the Department of Case Management. Your leadership will ensure the effective delivery of care management services, integration with related departments, and compliance with hospital policies and regulatory standards. Key Responsibilities:
Direct, coordinate, and supervise the operations of the Department of Case Management.
Monitor service activities and resource utilization, taking appropriate action as necessary.
Prepare and manage the annual departmental budget, ensuring alignment with hospital guidelines.
Develop and implement departmental goals, policies, and procedures.
Manage human resources functions, including staff selection, promotion, orientation, and performance appraisals.
Ensure compliance with hospital standards and regulatory requirements, directing quality improvement activities.
Coordinate discharge planning processes and ensure continuity of care with related departments.
Maintain effective relationships with community service organizations and public agencies.
Provide educational programs for hospital staff and students to enhance understanding of patient care.
Requirements:
Master’s Degree in Nursing (MSN) required.
Minimum of three (3+) years of progressive case management leadership experience in an acute care setting.
Current New York State Registered Nurse License.
