Position Summary:
Performs personal care, homemaking services under the supervision of a registered nurse. Assists with therapeutic exercise/ADL programs under the direction of a licensed therapist. Assists the patient in maintaining a safe, clean home environment through the accomplishments of specific tasks per the home health aide care plan. Performs duties in support of the CK Home Care’s and Hospice’s mission to ensure the highest quality of patient care in an economically sound and efficient manner.

* Assists patients with oral medication. Assistance is limited to reminding the patient to take his/her medication.
|
* Assists and supervises the patient with transfers and ambulation activities.
|
* Assists with therapeutic exercises as directed by the supervising Registered Nurse and/or Licensed Therapists.
|
* Demonstrates proper use of Universal Precautions and infection control with all patients.
|
* Promotes the patient’s mental alertness through conversation, reading, games and other activities.
|
* Encourages the patient to become as independent as possible within his/her limitations.
|
When assigned to the Inpatient Hospice Unit, adheres to the hospice restraint and seclusion policy and procedure in providing care and services.
|
When providing care to hospice patients, contributes to the comfort and dignity during physical and emotional care and services
|
As a Hospice Aide, participates as a member of the Interdisciplinary Group (IDG)
|
* Provides appropriate members of Home Health/Hospice Team with accurate reporting and recording of changes in client’s physical, mental or emotional status through documentation and verbal communication in a timely manner in accordance with agency policy and procedure.
|
* Assists with care and maintenance of equipment and supplies. Reports any unsafe situations/products to the supervisor immediately.
|
Demonstrates basic safety techniques in performing job tasks
|
* Maintains productivity standards of CK Home Health/Hospice
|
* Completes all required documentation accurately and submits to office by 8 AM the next morning.
|
* Participates in the Performance Improvement program in the department.
|
*Maintains adherence of attending mandatory 12 hours of in-service education during the calendar year.
|
Dress code compliance maintained.

Maintains adherence to privacy and security regulations of CK Home Health & Hospice

Education and Training:
High School Graduate or equivalent
Successfully completed one of the following:
- A training program and competency evaluation as specified in regulation.
- A competency evaluation program that meets the requirements in regulation.
- CNA (Certified Nurse Aide)-A nurse aide training and competency evaluation program approved by the State as meeting the requirements of § 483.151 through § 483.154 of this chapter, and is currently listed in good standing on the State nurse aide registry.
- A State licensure program that meets the requirements of paragraphs (b) and (c) of this section.
Mandated and Permissive Reporter Training required
Experience:
Skills: Reading, writing, typing, verbal clinical reporting, use of a computer for electronic
medical record documentation
Reading and recording Blood Pressure, Pulse, Temperature and body weight measurement
Basic infection control practices
