Formal Education Required: a. High School Diploma or equivalent
Experience & Training Required: a. Completion of approved Certified Community Health Worker Training Program b. CPR
3. Other Skills, Competencies and Qualifications: a. Certification through Ohio Board of Nursing b. Basic computer skills c. Must pass background check and drug test d. Knowledge of basic medical terminology e. Ability to work with diverse communities and different races, cultures and classes f. Ability and willingness to provide emotional support, encouragement and motivation to clients g. Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity h. Ability to obtain and record vital signs, and recognize changes in health status i. Second language skill is a plus

The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials.
