Under the Department of Pharmaceutical Services, Cardiology Ambulatory Pharmacist provide pharmaceutical care to patients at UCSF Ambulatory Care Clinics. The Cardiology Ambulatory Pharmacist performs a wide range of pharmaceutical care functions to assure that drugs prescribed for and used by patients are safe and effective. These include, for example, obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drugs of choice and therapeutic plans, ensuring access to medications, educating patients on the proper use of medications and ensuring that patients understand cautions, adverse reactions, and interactions related to their therapy. The Cardiology Ambulatory Pharmacist will be part of an interprofessional team consisting of physicians, advanced practice providers, nurses, and other clinic staff. The Cardiology Ambulatory Pharmacist will be based primarily in the ambulatory care setting managing cardiology patients. The Ambulatory Cardiology Pharmacist may also supervise pharmacy technicians to facilitate insurance authorizations for medications. The Cardiology Ambulatory Pharmacist may include as required oversight of the Patient Management Program, clinical activities, and delivery of services.

Please note: The compensation ranges listed online for roles not covered by a bargaining unit agreement are very wide, however a job offer will typically fall in the range of 80% - 120% of the established mid-point. An offer will take into consideration the experience of the final candidate AND the current salary level of individuals working at UCSF in a similar role.
For roles covered by a bargaining unit agreement, there will be specific rules about where a new hire would be placed on the range.

- Completion of PGY2 Ambulatory Pharmacy Residency in relevant area or at least 3 years of equivalent experience similar clinical practice (= 70%)
- Board certification (e.g., BPS)
- Experience with navigating billing options for pharmacist care
- Experience precepting and evaluating pharmacy learners (e.g., residents and students)
- Doctor of Pharmacy Degree from an accredited college or university or equivalent experience
- California Board of Pharmacy License or license eligible
- Completion of a PGY1 Pharmacy Residency or equivalent experience
- Excellent interpersonal skills
- Excellent written and oral communication skills to provide information clearly and concisely to, and to elicit information from, individuals with varying levels of ability to understand, including those who are ill or distressed, or who may not speak English.
- Computer skills to facilitate clinical patient data medication order entry
- Knowledge of clinical guidelines relevant to practice area and area of specialty
- This position requires flexibility to practice at all UCSF Medical Center locations
