As an Aging Care Manager 3, your primary responsibility is completing risk assessments and developing care plans to eliminate or reduce that risk for persons reported in need of protective services. Work includes obtaining information from consumers and caregivers to complete risk assessments, assessing all available information, making determinations, and developing plans with consumers. You will have the opportunity to provide services, investigate sites for clients at risk, and initiate procedures for court related intervention and guardianship cases. Participating in supervision, scheduled meetings, annual enrichment and other trainings will be required.

Work Schedule and Additional Information:

• Full-time employment.
• Work hours are 8:30 am to 4:30 pm, Monday - Friday, with 30-minute lunch.

QUALIFICATIONS

Minimum Experience and Training Requirements:

• One year as an Aging Care Manager 2 or Aging Program Assessor; or
• One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
• An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.

• You must be able to perform essential job functions.

Legal Requirements:

• You must pass a background investigation.
• This position falls under the provisions of the Older Adult Protective Services Act.
  • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
• This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
