Under the general direction of the Program Director, the Preventive (FC) Supervisor shall serve as a line supervisor. FC Supervisors coach, support, and guide case planners through the implementation and delivery of the Family Connections model. All components and stages of the Family Connections model will be followed, beginning with the one business day contact, going up to the evaluation of change. FC Supervisors will always be assessing for risk and safety factors. They will ensure that we provide quality services to families. FC Supervisors are to supervise teams, as assigned and other job-related task and duties. FC Supervisors will ensure that all fidelity measures for Family Connections are met. This includes keeping up to date on the new timeline that has been created for them which encompasses all of the Family Connections documentation. FC Supervisor’s attendance is a critical factor as it sets an example for workers, as well as providing stability for the units. However, their schedule must include flexibility as this is not always a 9-5 position.
Responsibilities
Supervisors will ensure that one hour of change focused intervention is conducted weekly with each family; constituting of 3-4 contacts before Service Plan is created and biweekly contacts thereafter.
Supervisors will ensure that case planner’s progress notes are entered within 5 business days and follow the Family Connections template. Each case will be reviewed with case planner a minimum of once per month. Progress notes will be reviewed in preparation for case review. It is the Supervisor’s responsibility to review the weekly contact grid to make sure all fidelity measures are being met and coincides with contacts entered into PROMIS, as well as every child being seen.
Supervisors will provide individual staff supervision once a week. This supervision will be documented in two ways; through case review and the individual log, (focusing on staff development), which encompasses the coaching methodology of Family Connections. Supervisors will also conduct weekly group supervision for their unit.
Supervisors will enter supervisory progress notes into CONNX on a monthly basis. They will follow many of the Family Connections criteria, as well as ACS mandates.
Supervisors will review all FASP’s prior to submission to ACS. They will ensure that they are completed accurately and submitted on time. Supervisors will ensure FASP submission for their unit adheres to internal timelines.
Supervisors will provide direct services to families within their unit when deemed necessary.
Supervisors will ensure that the intake process is followed through within correct timelines. Once a case is assigned the case planner must make contact within 24 hours. Supervisor must ensure that all intake forms and appropriate documentation is completed.
Supervisors are expected to communicate with ACS personnel, case managers, PPRS liaison officers, school personnel, court officials and other mental health professionals, as needed.
Qualifications
Master's degree in Social Work or Mental health Counseling.
3 years post master’s experience at a Preventive Service agency or 5 years’ experience at a human service agency.
Excellent interpersonal and relationship skills.
Minimum three years of demonstrated successful supervisory experience preferred.
Ability to work effectively as a member of a team.
Good planning, organization and priority-setting skills,
Demonstrated ability to work with individuals of diverse backgrounds and lifestyles.
Starting Salary $70,000
