Job Overview:
The position of Program Director/Advanced Practice Provider (APN or PA) for the Community Medical Center Health Lives Program is focused on overseeing the program and caring for patients primarily with Heart Failure, Pneumonia, COPD, and AMI after discharge from the hospital. As a clinician, the APP will support the clinical management of the patients admitted with a one of the program primary diagnosis and will work with the patient s team of medical providers inclusive of the Cardiac/Care Navigators, and interdisciplinary care team to ensure patients are being managed with guideline directed medial therapy, are meeting goals identified for timely discharge, education, and safe discharge with appropriate follow up. Primary care delivery model will be via telemedicine for patients managed through the Healthy Lives Center. This position will work alongside the AVP of Cardiovascular Services, AVP of Business Development, and the Chief Medical Officer to standardize the care of admitted patients, achieving programmatic goals and metrics for quality, safety, and efficiency, developing clinical protocols, and implementing standards of practice. The APP will be experienced in the multidisciplinary management of these patients and focused on Transitional Care Management and reduction of hospital readmissions. The APN will be clinically active in direct patient care and round in a multidisciplinary manner according to best practices, and be responsible for organizing and overseeing the care team
The role of Program Director will focus on the day to day management of the program, selecting and training administrative and clinical staff, and delivering on hospital and individual care goals. The PD will develop and maintain strong professional relationships with our community and employed medical staff and partner with our hospital leadership to grow the volume and improve the quality of the service line. This position performs the job functions in accordance with the mission, vision, values and policies of Community Medical Center and the RWJ Barnabas Healthcare System.
Qualifications:
- Master s degree in Nursing or Physician Assistant required.
- Registered Professional Nurse or Physician Assistant with current NJ state license; CDS and DEA.
- CPR certification.
- NP or PA License/Certification.
- At least 3-5 years of knowledge/experience in principles of management, accounting, problem solving.
- At least 3-5 years of progressive operational experience in key ancillary and/or support service areas.

Essential Functions:
- Organize and participate in daily multidisciplinary teaching/patient rounds for program patients and be available either in person or by phone throughout the day.
- Work with and communicate with attending physicians of record regarding issues related to patient care as the need arises.
- Develop and apply pathways and protocols of care as developed in conjunction with the Chiefs of Service, Chief of Hospitalists, and other clinical leaders.
- Be responsible for implementation, follow-up, and maintenance of quality and performance metrics and initiatives related to care of the program patient and readmissions.
- Ensure that care of the program patient is in accordance with best practices and national guidelines.
- Ensure and foster clear lines of communication between members of the multidisciplinary team, including nursing, attending physicians, resident staff, and other physician leaders.
- Meet with the CMO, AVP of Cardiovascular Services, and the Chiefs of Services monthly to discuss operational and clinical issues.
- Maintain a close relationship with physician peers in order to ensure seamless patient care.
- Lead key initiative and goals aligned with the CV service line strategic plan, to ensure successful planning and care in the program patients.
- Participate in the hospital Multidisciplinary committee for review of program patient readmission issues as required.
- Collaborate with nursing unit leadership on admission, throughput, and discharge planning.
- Participate in peer review as the need arises.
- See program patients in the OP virtual clinic setting for follow up post discharge, symptomatic patients in need to evaluation and possible treatment. Will communicate with patient s primary cardiologist any adjustments made or proposal to changes in care

- Develop operational practices, workflows, and best practice to deliver high quality patient care.
- Recruit and lead clinical and administrative support staff including hiring, training, performance management of all staff.
- Develop all policies and procedures for the virtual care program.
- Accountable to set policies and procedures and ensures all services meet the goals and objectives of Community Medical Center
- Accountable to drive, maintain a High Reliability culture and for operational aspects including quality outcomes, and patient experience
- Accountable for compliance with CMS, TJC, NJ DOH, OSHA, ACR, NJDEP, CAP and AABB
- Accountable for operational aspects including efficiency and budget including productivity, operational and capital budgets
