The Bilingual Care Coordinator provides intensive clinical care coordination to the youth and family as well as counseling and behavioral/emotional support. The primary role of the Bilingual Care Coordinator is to coordinate care among all entities involved with the child, ensuring everyone is aligned with the goals. Additionally, the Bilingual Care Coordinator helps build natural supports and enhances the treatment team. They coordinate and are in change of the planning for children who are referred primarily through their parents . Services will be provided almost exclusively in the community in families’ homes, group homes, foster homes, schools, etc. Ensuring that all parties collaborate toward stability for the child and family, and establishing a family team through team meetings to help the youth and family achieve their goals. Clinicians engage clients in meaningful and authentic relationships while providing a safe, predictable, and positive environment that aligns with our clients’ values and cultures.

Seneca serves Santa Clara County children/youth by offering a focus on managing mental and behavioral health needs and increasing placement stabilization. This program provides culturally-competent, team-driven, and strength-based unconditional care for children and families who struggle with profound needs and difficulties. Katie A. Services services dually focus on the child and family team meeting process that focuses on building a strong team to collaboratively address the needs of the child and family. Services are provided at the program site and in the community, including schools, and family homes.

Responsibilities
Promptly meet with clients and caregivers on a regular basis to further treatment goals utilizing culturally informed and best practice modalities.
Coordinate with multi-disciplinary teams for the purpose of care planning and support.
Collaborate with agency staff, caregivers, other professionals and interested parties to formulate and implement individual client care plans, behavioral programs, and interventions.
Cultivate and maintain relationships with community members and providers, as needed.
Complete assessment and evaluation process of clients in order to develop and implement the care plan.
Provide assistance in case planning and consultation to team members on clinical issues, as well as casework issues, as appropriate.
Chart mental health notes, care plans, outcome measures, incident reports, and other documentation accurately and in a timely manner.
Maintain availability for crisis intervention services, such as on-call rotation, if applicable.
Participate in individual and/or group supervision.
Attend all required trainings.
Comply with all child abuse reporting and Medicaid/Medi-Cal requirements in accordance with standards set by Seneca.
Maintain strict confidentiality of client and business operations information.
Commit to and engage in self-evaluation as well as organizational goals, strategies, and systemic practices regarding diversity, equity and inclusion.
Utilize crisis communication and de-escalation techniques as per Seneca's EQ-4 model training. This may include intervening with the clients' behavioral program through physical management of the clients, if applicable. Training in additional crisis intervention modalities such as CPI may be required.
Qualifications
REQUIRED
Master’s degree in social work/psychology/counseling
Current licensure or eligibility for licensure with the applicable regulatory agency responsible for licensing, examination, and enforcement of competent mental health practice standards.
At least 21 years of age
TB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirements.
Valid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policy.
Proof of up-to-date COVID-19 vaccination or an HR-approved medical/religious exemption
PREFRRED
Experience working with children/youth and families with complex and enduring needs
Experience implementing family-focused services
SKILL OF AN IDEAL CANDIDATE
Ability to work effectively with a diverse population and workforce
Strong organizational, time management, and writing skills

Benefits
Starting at $88,400 to $102,400 per year
As of July 7th, this starting range will increase to $91,232.00 - $105,232.00
Actual salary is dependent on creditable experience above the minimum qualifications for the role
Additional $4000 salary increase for BBS licensure
