Posting Summary | Rutgers, The State University of New Jersey, is seeking a Patient Representative for the Holocaust Survivors Program and Veterans Smile Program at the Office for Clinical Affairs within the Rutgers School of Dental Medicine (RSDM).
The Patient Representative serves as the liaison for all new patients to the departments or specialty programs. The Representative will guide patients though RSDM, ensuring appropriate appointments are given in a timely manner through treatment plans agreed upon. The Representative will assist with access issues, develop relationships with referring providers, internal and external and ensure the complete and efficient management of all referrals, including the acquisition of necessary patient-care materials and medical records. In addition, the Representative will track all new patient request, provide monthly statistics and referring patterns.
Among the key duties of this position are the following:
Initial Referral: - Provides information about RSDM’s services and expertise available.
- Elicits patient clinical information to allow for appropriate scheduling of referrals. Refers to guidelines on how to schedule emergent/urgent requests or specific diagnoses.
- Encourages referrals and schedules new appointments as indicated.
- Provides directions including parking and mass transient accessibility.
- Assists in patient registration and assignment.
- Verifies secondary insurance eligibility, and provides guidance around referral and managed care issues, and information about other prior authorizations that may be needed for testing and procedures.
- Provides all patient material.
- Maintains database of new patients and provides monthly statistics on volume and procedures.
Representative Duties: - Liaison between patients and RSDM dental students/post-graduate residents/faculty.
- Answers patient telephone calls. Correctly processes information for appointment scheduling, answer questions, and assist with problem solving. This includes but is not limited to triaging emergent/urgent phone calls, inquiries related to the availability of appointments, scheduled appointment dates and times, parking, direction, and general practice information.
- Reviews all patient demographic information, insurance information and referral numbers. If any discrepancies, updates information as appropriate.
- Helps patients arrive at scheduled appointments.
- Greets patients and courteously and efficiently processes their information or refers them to the appropriate location.
- Facilitates all patient appointments with specialty clinics and outside labs to expedite care.
- Schedules appointments accurately, accommodating the patient as much as possible. • Actively identifies and addresses barriers to care that might keep the patient from receiving timely and appropriate treatment.
- Handles all patient concerns in a respectful and understanding manner.
- Treats all patients with dignity and professionalism.
- Ensure continuity of care is maintained.
Communication with Clinical Team: - Communicates with faculty, dental students, and residents in a professional manner.
- Relays messages to clinicians and follow-up as indicated by supervisor.
- Follows all dental protocols to accurately schedule patients according to diagnosis and referral.
- Provides all follow-up with patient and dentist as appropriate.
- Ensures completed consults are forwarded to referring dentists in timeframe established.
Records Management: - Acquires all materials, including films, reports, and laboratory reports necessary for each new patient visit. Ensures these records are available for clinicians prior to appointments, if requested, to determine appropriateness of referral.
- Processes all patient information accurately and expeditiously.
- Accurately and efficiently prepares all new patient records as dictated by practice standards for scanning into the electronic medical record.
- Advises patients about records and forms needed to bring to each visit.
