Identify patients at nutritional risk. Assess assigned patients and completing accurate documentation. Implement a nutritional care plan based on the assessment and needs of the patient. Provide nutrition education to patients/family members. Conduct community and/or patient education classes. Advise Physicians and all related Medical Personnel in areas of nutrition.
