Primary Objective: Serves as the primary contact for individuals and families served as well as the third party administrator for DCBDD's Family Support Services (FSS) program.
Job Standards
Minimum Qualifications: Bachelor’s degree in social work, counseling, education, health and human services, law enforcement, behavioral science, or a related field and two (2) years’ experience working with individuals with developmental disabilities; Master’s degree in related area and four (4) years’ experience working with individuals with developmental disabilities preferred.
Must be able to obtain upon appointment and maintain DODD Service and Support Administration Certification per OAC 5123-5-02.
Must possess a valid State of Ohio Driver's License and acceptable driving record as stipulated by the Board’s insurance carrier.
Physical Requirements:The physical requirements of the position are identified as light work, which may require the lifting of up to 25 pounds occasionally.
The physical activity of the position is manual dexterity, talking, hearing, reaching, and navigating your environment.
Ability to operate standard office equipment in the performance of essential functions (e.g. copy machine, fax machine, personal computer, and telephone.).
Other Requirements: Successful completion of a criminal records check (BCI and FBI) and registry/database check as required by DODD, driving record checks, and DCBDD receiving favorable reference reports.
Schedule may need to be flexible based on DCBDD operational need and/or the individual or families served.
Essential Functions
Serves as the Family Support Services (FSS) liaison between DCBDD and the third-party administrator of the program; responds to requests and questions for information from the third-party administrator; consults and provides feedback to the Senior Director of Programs and Services regarding programmatic decision-making of the FSS program and complex service issues.
Determines enrollment of eligible individuals in FSS or service and support administration; inactivates an individual when no contact is made with DCBDD or services are not requested.
Collects information to identify possible services and/or community resources for individuals who have been referred to FSS; provides information and referral to appropriate community services.
Monitors FSS referrals for use of services; creates and maintains records and statistical information to reflect program and administrative activities for the FSS program.
Administers Waiver Waitlist Assessment; acts as authorization contact for individuals placed on Waiver Waiting List; assures established timelines for Waiver Waitlist are followed.
Tracks the eligibility redetermination timeline and completes document review for individuals enrolled in FSS.
Completes live targeted case management (TCM) notes to document all services delivered and service billing; maintains documentation and individual files.
Develops trainings based on identified areas of need, including but not limited to, curriculum, instructional materials, visual aids, training manuals, etc.
Supports Programs and Services by conducting a variety of assessments, including but not limited to, Extraordinary Care Instrument (ECI) and Charting the Life Course.
Documents direct and collateral contacts and other activities as prescribed in DCBDD policy and procedure.
Collaborates with Communications and Community Engagement Department regarding available community resources.
Provides Service and Support Administrator (SSA) backup supports based upon the operational needs of the organization.
Schedule may need to be flexible based on needs of the agency and/ or person served.
Regular attendance, as governed by and in accordance with applicable rules, regulations, procedures and laws, is regarded as an essential requirement of this classification.
Adheres to all ORC, OAC, and DCBDD policies and procedures.
Performs other related duties as assigned.
