Benefit Case Manager Overview:
Under the supervision of the Guardianship Benefit Coordinator, this position serves as a Benefits Case Manager for the Guardianship Program. Responsible for the applications and recertification's of public and private financial and medical benefits for the clients of the program. Coordinates with internal and external stakeholders to research, verify, establish, and maintain benefits for the program’s clients.
Duties & Responsibilities:
Recertifies all Medicaid, Medicare Savings Program, and Supplemental Nutrition Program benefits for the wards of the program.
Coordinates with program staff to secure the required documentation.
Compiles all pertinent information to complete the application and recertification process.
Logs in all financial and medical benefit applications.
Coordinates with internal stakeholders such as the asset management team, fiscal department, and program staff to research and establish clients’ benefits.
Evaluates, assists, and completes enrollment for eligible Medicare programs.
Completes applications to become a fiduciary and payee for pensions and Veterans Administration benefits.
Alerts case managers and supervisors of eligibility criteria and provides time frames for applications (initial applications).
Coordinates Medicaid reimbursement for eligible wards
Completes, e-files, and monitors legal applications.
Coordinates with the County Attorney’s Office Compliance Department
Writes RFPs, reviews, and reconciles payments.
Develops and maintains monthly reports.
Maintains knowledge of all counties, state, and federal assistance program requirements.
Stays abreast of changes and additions to benefits and eligibility.
Documents contacts and services for clients and maintains files and data for reporting purposes that adhere to fiscal department parameters.

Requirements
Education:
Bachelor’s Degree in Social or Behavioral Sciences, Public Health, Business or closely related field.
Experience:
One (1) year full-time paid work experience in Benefit Counseling and/or Benefit Case Management with eligibility determination.
Training and experience using a personal computer with a variety of software such as Microsoft Word, Microsoft Excel, Microsoft Outlook, and Case Management Software.
Licensure:
Reliable transportation with a valid driver’s license (Texas) and auto liability insurance.
Knowledge, Skills & Abilities:
Follows written and oral instructions.
Possesses excellent organizational skills.
Must be detailed oriented and self-motivated.
Must interact with all levels of staff.
Have excellent oral and written communication skills.
Problem solve and demonstrate critical thinking skills.
