As an RN Case Manager, you will perform comprehensive assessments upon admission, utilize established tools to identify high-risk patients, and ensure they receive the necessary support for a successful discharge. Key Responsibilities
Comprehensive Assessments: Conduct initial and ongoing assessments to identify patient needs and potential discharge planning requirements.

Requirements
Education: Bachelor’s Degree in Nursing (BSN) or a related field required.
Licensure: Current RN licensure in the state of New York.
Experience: Minimum of 5 years of clinical experience in ED, critical care, or med/surg settings within an acute care hospital.
Case Management: Previous case management experience preferred.
Knowledge: Familiarity with healthcare financial, regulatory, and payer issues; state, local, and federal programs; Milliman/InterQual criteria.
Certifications: PRI/Screen certification preferred; C.C.M. (Certified Case Manager) certification preferred.
