The diabetes patient education nurse functions independently and works collaboratively with the patient, caregivers, and multidisciplinary team to develop and implement high quality diabetes education for patients through assessment, instructions, intervention, counseling, and discharge planning. This position includes planning, coordinating, and evaluating diabetes care for hospital acute adult, emergency, OB, psychiatry, pediatric, post-acute, and rehabilitation areas. Ability to assess for motivation, readiness to learn/make behavior changes, attitude toward learning and preferred learning style; assessments reflect special considerations for all age groups. Understand basic knowledge of diabetes, health beliefs, impact of social, economic, and cultural aspects/circumstances that affect diabetes self-management skills. Ability to strongly communicate and serve as diabetes patient advocate.

CERTIFICATION & LICENSURE:
RN-Registered Nurse of California
BLS-Basic Life Support Healthcare Provider or ACLS-Advanced Cardiac Life Support
Certified Diabetes Care and Education Specialist (formerly Certified Diabetes Educator) or working toward completion of certification within one year of hire)
TYPICAL EXPERIENCE:
3 years of experience in a clinical specialty area or education preferred. Diabetes exp preferred. Bilingual Spanish capability highly preferred.

Pay Range is $83.32 to $109.98 / hour
