Job Abstract

Summary:

Provides quality nursing care through the application of the nursing process using evidence based practice focusing on excellent outcomes and patient experience.

Primary Position Responsibilities:

Facilitates patient care

Documentation of care

Education of patients and families

Patient Advocate

Maintain patient safety

Other duties as assigned

The LPN assists with ongoing assessments, performs interventions, prepares for procedures, monitors, observes and reports changes or findings to the RN.

Qualifications:

Minimum Education

Required: Nursing Diploma, Graduate of an Accredited LPN Program

Preferred: Associates Degree

Minimum Years of Experience

Required: 1 year of Medical Surgical, Home Health, Nursing Home, SAR or Wound Care

Preferred: 1+ years of Medical Surgical, Home Health, Nursing Home, SAR or Wound Care

License Registry or Certification:

Required: Current NJ LPN License, Current CPR Certification

Preferred: Wound Care Certification

Knowledge, Skills and/or Abilities:

Required: Must pass initial and annual clinical competency based program by exam and skill performance as well as mandated in-service education.
