Assists with the daily functions, patient outreach, and identification of community resources for patients, patient counseling and education for the multi-disciplinary clinic.
Duties:
- Responsible for patient counseling and referrals for patient needs.
- Assists with coordinating patient care.
- Responsible for the on-going daily management of patient continuity of care for patients entering and going through the multi-disciplinary clinics.
- Organizes and assists with conducting community outreach on behalf of the Program.
- Maintains a customer service program to ensure a high quality service and customer satisfaction for patients in the clinical setting.
- Creates and conducts surveys for evaluating satisfaction of the users of the program.
- Participates in and initiates advancement of hospital, community and patient relations.
- Survey and follow-sup with referring providers to ensure high quality customer service.
- Meets with patients to ensure customer satisfaction with the Program and hospital experience.
- Utilizes and/or creates databases for tracking and following patients through out-patient clinical settings.
- Organizes and analyzes program data on a regular basis.
- Creates and maintains referring physician informational databases.
- Maintains all documentation and files on behalf of the program.
- Performs various duties as needed to successfully fulfill the function of the position.
Required Education: Bachelor's Degree in Health Education, Social Work, Psychology, Nursing, or health related field AND:
- 12 months experience in a medical or clinical care setting.
Equivalency/Substitution: Will accept 48 months related experience in lieu of the Bachelor's Degree for a total of 60 months experience.
Certifications:
- A license from the State of Oklahoma related to the required health field.
