Responsibilities
Conducts enrollment assessments to determine clinical eligibility.
Completes UAS - NY Assessment instrument.
Maintains frequent communication with the Home Care Department and Care Management to keep ArchCare Senior Life a primary option for care delivery.
Identifies any barriers to implementation of Plan of Care.
Communicates with the Care Team in conveying information to facilitate any concerns, or answer questions.
Completes UAS - New York Assessment instrument as assigned to determine continuing eligibility and / or identify any needed changes to Care Plan.
Performs other related duties, as required.
Job Type: Per Diem
Salary: Fee-for-service structure. $225 per assessment, $250 per new enrollment
Qualifications
Registered Nurse, currently licensed and registered to practice in New York State.
UAS-NY Certified
Bachelor's Degree in Nursing preferred.
Experience in Home Care, case management, or long-term home care.
Demonstrated skills in planning, organizing and directing the work of others required.
Experience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments.
