The hospital also has services in intensive outpatient and partial hospital program services. www.kingwoodpines.com POSITION SUMMARY: Kingwood Pines Hospital is seeking a dynamic and talented Step-Down Navigator. This position will be responsible for meeting with all eligible patients to discuss the benefits of step down services as part of their continuum of care treatment. Additionally, this role will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to eligibility. KEY RESPONSIBILITIES: Identifies and meets with 100% of eligible patients to discuss the benefits of appropriate programming options for patients Ensures all aspects of the step down process are followed: identification of eligible patients, insurance verification, treatment team plan approval, etc Assist with discharge planning for PHP/IOP patients already enrolled in the program Maintains electronic data collection process. Provide weekly report to Business Development, CEO and Clinical Services Director. Thorough understanding of step-down process and familiarity with involved departments required. Thorough understanding of inpatient workflow process, as it pertains to case management and discharging planning preferred. EDUCATION: High School diploma or equivalent required. Bachelor's degree in Social Services or Patient Care related field preferred. CERTIFICATION: Current CPR, Physical and Verbal Crisis Intervention Certification. Training may be obtained during new hire orientation.
