Collaborates with the Stroke Nurse Clinical Coordinator and the medical director (s) of the Stroke Center in strategic initiatives associated with the goals of the Stroke Center. This role serves as a member of the Stroke Core Team and is focused on data management and performance improvement programs which are based on evidence based practice as outlined by AHA/ASA guidelines. Oversees data management including data collection, abstraction, analysis, tracking, and reporting; responsible for creating requested reports, presentations, and details for data abstraction/analysis. Maintains responsibility for coordinating clinical information and data to support clinical quality performance measurement and improvement. Collaborates with the Stroke Nurse Clinical Coordinator for performance improvement projects; works with administrators, physicians, nurses and staff to ensure an efficient, accurate data abstraction and analysis process which supports evidence based practice and performance improvement projects. Collaborates with all Stroke Neurovascular Center leadership, Clinicians, Stroke Coordinators, and Performance Improvement Coordinator (s) in all aspects of data management and reporting related to performance management, departmental initiatives, and regulatory requirements. 3.1 Oversees data reporting for performance management requirements to all regulatory and accrediting agencies. 4. Collaborates with Stroke Center Leadership and the Stroke Nurse Clinical Coordinator in the identification, recommendation, implementation, and evaluation of best practices using data driven methodical approaches. 4.1 Collaborates in, and facilitates the development of data measures and reports. 5. Demonstrates high level knowledge and skills that reflect expertise in communication, customer service, professionalism, problem solving, resource management, results orientation, teamwork, cooperation, organizational expertise, leadership, and data management. 5.1 Serves as the liaison for the Bridgeport Hospital Stroke Center data management and associated performance improvement functions. Qualifications EDUCATION Bachelor's degree in Nursing is required. Masters preferred. EXPERIENCE Experience in acute care nursing, specific to the neurovascular service line including experience in data outcomes, management, analysis and report creation. LICENSURE Licensed Registered Nurse in the State of Connecticut required. SPECIAL SKILLS Background in neuroscience/stroke care and rehabilitation. Must possess understanding of clinical processes, organizational dynamics and process improvement. Must be self motivated, goal oriented and able to work independently. Must have the ability to develop relationships and work collaboratively and effectively in partnership with physicians, clinical staff, hospital departments and outside organizations. Must have excellent assessment, communication and organizational skills with the ability to manage and follow through on multiple priorities. Must have ability and/or experience collaborating with senior leaders and physicians on business strategies and tactics. Expertise in the use of Microsoft Office including Power point and Excel, Epic including navigation of EMR, data abstraction and knowledge of process improvement techniques. Expert knowledge of information systems and external databases associated with accreditation and regulatory requirements for quality of care, treatment, evaluation, and public reporting. Excellent critical thinking, analysis and assessment skills for successful process improvement planning and monitoring. Position requires recognition as a leader demonstrating excellent interpersonal, communication, negotiation, problem solving and customer service skills and ability to lead and motivate work teams.
