Requirements:

• Current Licensed Practical Nurse (LPN) in the State of Louisiana
• Minimum of 1-2 years medical office experience
• Computer skills and data entry experience
• Goal-oriented and excellent communication skills
• Good command of MS Office
• Medical, home health or hospice office experience required

Job Duties:

• Conduct initial screenings and intake assessments of new clients
• Gather essential client information such as medical history, insurance information, and contact details
• Coordinate with other members of the team to ensure proper care settings for each client
• Communicate with clients and their families regarding the intake process and any necessary documentation or information
• Enter client data into electronic medical records system and update as necessary
• Ensure compliance with HIPAA regulations and maintain confidentiality at all times
• Monitor and track intake process and provide regular updates to team members
• Collaborate with other departments to ensure the smooth flow of information and coordination of care
• Participate in continuing education and training to maintain knowledge of best practices and regulatory requirements.
• Scheduling of clinicians in the field as needed (upon training)
