Summary:
The hospice chaplain provides spiritual/religious and emotional support, as well as counseling to patients and their families, and at times, staff. The chaplain demonstrates competency in dealing with grief, death and the process of dying. Additionally, the chaplain helps care recipients to draw upon their spiritual and other support resources in coping with and identifying meaning in the time surrounding the end of life. Essential interventions the chaplain utilizes include supportive and active listening, pastoral counseling, crisis intervention, acceptance building, and prayer. The chaplain will also coordinate care with the Interdisciplinary Team, and at times with appropriate clergy in the community. The chaplain acts as a resource and liaison with patient/families and the hospice team.
Patient Care:
Provide spiritual and emotional support as well as counseling to patients and their loved ones.
Interact with hospice team members to identify and address patient care needs and develop the plan of care.
Assess all cases within five days of start of care.
Ability to work with people or any or no faith tradition, as well as different lifestyles, cultures, beliefs, and values.
Provides documentation in the online chart utilizing good written communication skills.
As needed, the chaplain utilizes such interventions as: active listening; coordination or facilitation of appropriate rituals; empathetic questions; empowerment, end of life decision making support; legacy identification; life review; ministry of presence; prayer; spiritual/religious counsel; support navigating family conflict.
Requirements
Minimum Qualifications:
Theological master’s degree from an accredited school.
Two (2) units of Clinical Pastoral Education (CPE).
One (1) year healthcare chaplaincy experience.
Demonstrates good verbal and written communication skills.
·Ability to work competently with technology (computers, tablets, smartphone, etc.)
Ability to function as member of interdisciplinary group.
Preferred Qualifications:
Two (2) or more years experience as a hospice chaplain.
Four (4) or more units of Clinical Pastoral Educate (CPE).
Coursework in: bioethics; counseling; grief, loss, death, and dying.
Ordained, credentialed, or commissioned according to the practices of a legitimate and recognized religious organization or faith tradition.
