Formal education requirements: a. Masters degree in Social Work b. LISW in the State of Ohio
Experience and Training: a. Three (3) years of social work experience. b. Experience working with patients who have behavioral health issues or cognitive decline preferred
Skills, Competencies and Qualifications a. Ability to travel from location to location throughout the workday. b. Demonstrates basic skills as well as navigation and application of electronic devices and platforms c. Maintains confidentiality of all patient information d. Requires interpersonal skills sufficient to interact effectively with patients, their families, interdisciplinary caregivers, peers, subordinates, and supervisors. e. Possesses sound knowledge of community resources/agencies that serve the frail and behaviorally complex populations f. Ability to communicate effectively, accept and institute change and work independently as well as collaborate with others g. Ability to effectively interact with populations of patients/customers with an understanding of their needs for self-respect and dignity

The salary range on this job posting/advertising is base salary exclusive of any bonuses or differentials. Many factors, such as years of relevant experience and geographical location are considered when determining the starting rate of pay.
