Responsibilities
The Nurse Care Coordinator serves in an expanded nursing role to collaborate with patients and their primary care providers to offer a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. The Nurse Care Coordinator:
Assesses, plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient’s health status.
Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the populations.
Develops systems of care that monitor patient progress and promote early intervention in acute care situations.
Works effectively with other members of the health care team to optimize interventions.
Manages utilization and practice metrics to further refine the delivery of care model to maximize clinical, quality, and fiscal outcomes for identified populations.
Qualifications
Education and Experience: Associate's Degree, Diploma, or Certification in Nursing and over 6 months to 1 year office RN experience required BSN preferred
Licenses and Certifications: Basic Life Support (BLS), Upon hire, required Registered Nurse or Licensed Practical Nurse in PA – Licensed, Upon hire, required
