Job Description:
*REMOTE POSITION*
The Telehealth Program Supervisor functions as a supervising authority for as part of the broader ElaraConnect Platform. The incumbent is responsible for the generation and development of new leads for home care, both skilled and non-skilled, to improve patient access to our suite of services provided at Elara Caring while ensuring all home care needs are met appropriately in a cost-effective manner. This position will assist in providing education and training related to patient engagement and lead the generation of referrals for new employees during the precepting phase of new hire orientation. This position aids in leadership of the broader Check-In Program roles and mission.

What is Required?
- Associate degree in Nursing
- 4 years in a health care setting, medical terminology knowledge, and leadership experience is required
- Excellent interpersonal skills and ability to communication professionally and effectively with staff
- Demonstrates problem solving, organizational, and attention to detail skills
- Ability to communicate program goals and progress with directors, stakeholders, and C-Suite
- Knowledge of Medicare and Medicaid payment guidelines and third-party payors is preferred
- Knowledge of telehealth and HomeCare HomeBase is a plus
- Reliable transportation to perform job tasks
You will report to the Director of Patient Services.
