Your responsibilities include but are not limited to:
• Ensuring that the patient understands findings from the standard assessment procedures, especially the connections between adverse findings and lifestyle choices and environmental exposures.
• Helping devise and implementing strategies to improve the patient's health, as recommended by the physician and nutritionist.
• Maintaining a relationship with the patient, troubleshooting problem areas, revising plans as needed, and generally providing professional support and encouragement.
• Reporting back to the clinical team on the patient's success and ensures the patient returns to the clinic when needed.

Responsibilities:
- Acts as an integral member of the clinical care team and to educate patients about findings and recommendations.
- Coaches patients about behavior changes to improve health status, reduce health risks, and improve quality of life.
- Assists with development and coordination of shared medical visits.
- Assesses readiness to change and implement actions to assist patients in reaching their goals.
- Empowers patients to become integral partners with their clinical care teams by teaching self-management skills and encouraging personal insight and self-activation.
- Develops customized (personalized) care plans for each patient including goals and action steps.
- Provides health and wellness education utilizing a variety of strategies including group presentations, support-group facilitation, creative use of media, newsletter contributions, etc.
- Monitors real-time participant data to view progress and/or challenges.
- Prioritizes and organizes daily workflow as well as other assignments/projects to meet commitments and deadlines.
- Other duties as assigned.
Education:
- Bachelor's Degree in one of the following fields: Health Promotion, Nutrition/Dietetics, Public Health, Exercise and Sport Science, and/or Physical Education.
Certifications:
- Certification from an accredited professional health coach training program preferred.
Complexity of Work:
- Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
- Must be able to work in a stressful environment and take appropriate action.
Work Experience:
- Minimum 2 years of experience in a healthcare setting/coaching environment.
- Expertise in helping patients prevent and/or improve management of lifestyle-related chronic conditions using strategies such as smoking cessation, weight management, dietary interventions, stress reduction, and exercise.
Personal Protective Equipment:
- Follows Standard Precautions using personal protective equipment as required for procedures.
