Job Description

The Lead Hospice Medical Social Worker provides clinical supervision for the hospice social work staff. The MSW serves as an advisor, consulting with the BSW’s and other social work team members, on assessing the needs of patients and families, developing and updating the social work portion of the plan of care and delivering care to patient and families. Supervision of the social work team must occur on a regular basis, based on the needs of the patient and skills of the social work team. Participates in the development of the plan of care, prepares clinical progress notes, works with the family, uses appropriate community resources, participates in the discharge planning and in-service programs and acts as a consultant to other agency personnel. This position may also provide clinical services to assigned patients including General Inpatient and Respite coordination, hospice consultations, supportive services, education, and bereavement support.

Education Requirements

• Master’s Degree in Social Work

Experience Requirements

• Two years of hospital/medical experience required and one year of hospice experience required.

Skills Requirements

• Competence to deal with patients and families of varying ages, stages of development, and at end of life.
