Focus of work is to insure PERMANENCY, SAFETY AND WELL-BEING. Social Service delivery is to be consistent with the philosophy of Good Shepherd Services, the function and tasks of the programs and in compliance with City and State mandates. Social Worker Reports to: DIRECTOR, GREEN HOUSE Social WorkerLocation: 2190 University Avenue, Bronx, NY 10453 Social Worker Hours: 35 Hours/Week; Exempt Social Worker Major Duties: Perform intake and assessment interviews. Provide ongoing individual and where applicable family therapy to program participants. Maintain a caseload of up to 8 residents plus trial discharge youth. Assist with stabilization, respite and crisis management. At intake, in collaboration with internal and external partners, begin the process of identifying permanent connections and formulating discharge planning. Formulate a written recommendation based on participant/family and team input. Promote permanent connections, including reunification, foster boarding home placement, adoption and alternative living arrangements. Assist in youth and family transitioning to next phase of their care post discharge. Collaborate with referring and community agencies to advocate for the needs of participant and family. Participate in meetings both weekly and as needed with youth development staff, and other staff for case collaboration and planning. Participate in weekly individual and group supervision as indicated. Attend internal and external meetings as indicated. Conduct home visits and escort youth to court and other appointments where indicated. Maintain mandated paperwork and recordkeeping in compliance with ACS, OCFS and GSS PEP Dept. Function as member of multidisciplinary treatment team. Participate in Family Team Conferences. Act as a liaison with Administration for Children’s Services (ACS), family court and appropriate community linkages. In collaboration with the Permanency Worker and ACS, make referrals for community services/housing as indicated Facilitate family visits, including visits with siblings in foster care. Prioritize permanency planning and prepare and submit court reports. Insure each program participant is either returned to family or linked with a caring adult. Effectively use and practice the Sanctuary Model tools: safety plans, community meetings, psycho-educational groups, team meetings, red flag reviews, treatment planning conferences (or similar) and self-care in the department/program to create a non violent and trauma-sensitive culture. Promote and model the seven Sanctuary Model commitments to create a non-violent and trauma-sensitive culture. Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an affective work environment. Support the implementation of the Missouri Approach in the program. Perform other duties as assigned. Social Worker Qualifications: Master’s degree in Social Work with license, or licensure eligible (LMSW) required. Minimum of one year’s experience in Child Welfare working with youth in residential setting and their families Clinical work with youth and families required; including crisis management. Proven participant sensibility and the ability to communicate with staff on all levels. Demonstrated competence in verbal, written, organizational, and prioritization skills. Capability to transition easily between working independently with a high degree of self-initiative and contributing as part of a team. Must be able to use public transportation systems, including subways and buses to attend various meetings and make home visits. Bilingual Englsih/Spanish preferred. Excellent crisis intervention, advocacy and problem solving skills. Ability to maintain certification in the program’s chosen behavior management/de-escalation method. Must have knowledge of Connections and Microsoft Word. Must have ability to work flexible schedule including some evenings, weekends and holidays. Must have a commitment to work from a strength-based, youth development perspective and trauma-informed perspective.
