Program Coordinator (150) Atwater, CA
Under direct supervision of Program Director. Provide support do direct care staff, encourage independence to all clients according to their abilities. Collaborate with care facilities and or family and CVRC to develop daily objectives and care plans for each client.
How will I contribute
Develop program plans for clients on caseload.
Interact with funding agencies, parents, and care providers.
Work with direct service staff to implement client goals and objectives.
Assist in the maintenance of the client database as it pertains to caseload.
Monitor client changes, communication and collaboration with care facilities
Monthly staff training, meeting CARF standards
Develop curriculum to meet the abilities of each client
Implement Restricted Health Care Plans created by care facilities and CVRC
Monitor medications brought into facility, proper documentation for medications for those with medications
Maintain inventory of ADP supplies (ordering arts and crafts supplies, toileting supplies) monitoring spending budget restrictions
Maintain cleanliness of building, reporting repair needs to Operation Manager
Maintain accurate and up to date client information within the database
Trouble shoot minor staffing/client situations
Assist staff with computer time keeping system
Monitor attendance of clients and staff
Communicate with care facilities and families regarding client attendance
Maintain flexibility, cover and support classroom needs
Assist with hiring process of potential staff
Staff evaluations
Staff corrective actions, implement and demonstrate KV expectations
Correspond/collaborate with HR on staffing needs
Client progress and issues documented in electronic tracking system
Provide direct services to consumers on an intermittent and or temporary basis.
Assist with development, implementation and monitoring of approved program budget
Electronic charting training
Perform other duties as assigned.
What I bring
Bachelors degree from an accredited college or university with emphasis in behavioral sciences or Associate degree and four yeas appropriate experience working with the developmentally disabled, mentally disabled, and physically disabled.
Class C Driving License
CPR, First Aid Cert
Computer literate and able to teach computer skills, self-motivated.
Clear and timely written and verbal communication with others.
Follow written and verbal directions.
Present information effectively, accurately and thoroughly in a style others can understand and act upon.
Listen to others information, ask questions that clarify the intended message, and respond to questions from manager, consumers, and outside organizations.
Written work is thorough and accurate.
Written materials are provided in a timely manner and in a style others can use
Must be adaptable. Must be physically and mentally fit to work with clients who are potentially dangerous to self or others in accordance with the Companys Physical and Mental Fitness standards. Ability to pass a health screening physical examination, drug screen and TB test.
Must meet state personnel licensing standards.
Must be willing to work within a culturally integrated workplace and be willing to respect human differences based upon race, age, disability, religion, sexual orientation, political persuasion, place of origin, color or any other condition that distinguishes people from one another.
