The Resource Navigator blends the mission of AHF with peer-to-peer experience in navigating high risk negative patients to supportive services. Utilizing their own health experience and a strong skill set in understanding and working through the healthcare system, as a Resource Navigator, you will work with high risk negative and PrEP patients that need additional services. Your compassion and ability to relate to patients provide a level of comfort and trust to our patients.
The Resource Navigator collaborates with the Public Health Team, Healthcare Center Staff, and Providers so that every patient receives the support they require and access to available resources. This will include providing education and assistance so that eligibility for the program is understood. Working directly within the Healthcare Center, the Resource Navigator is available to talk with every patient as needed.
By utilizing your own healthcare experience, you will meet our patients where they are, identify strengths and opportunities for growth and assist them in reaching their goals. With your collaboration and engagement skills, you will guide the patient through AHF’s supportive services. Your contacts with patients directly contribute to AHF’s success in meeting the deliverables of the Ohio Department of Health grant deliverables.
