Qualifications:
Current Licensed Practical Nurse (LPN) in the State of Louisiana
Long term care experience required
Medical facility auditing experience required
Knowledge of electronic charting system
Self-starter and the ability to recognize issues and solve them
Strong computer, medical records, communication and customer service skills
Great attention to detail
Job Duties:
Monitor medication administration to ensure that patients are taking their medications accurately
Report any hazards or concerns to the DON or other appropriate staff member
Participate in QAPI initiative to improve clinical processes
Review medical records to identify treatment plans or process errors or problems
Prepare reports on patient outcomes that are used to improve patient care and nursing practices
Work with clinical staff to develop, implement, monitor and update quality improvement initiatives
Develop and implement a system for monitoring the quality of care delivered by nurses and other staff colleagues
Observe patients to identify any changes in their condition or behavior that may indicate a need for interventions
Audits throughout the facility will be required
Ensure all admits and readmit orders are correct
Confirm all resident appointments are scheduled timely
Monitor all immunizations, including checking the links system and ensure all immunizations for new admits comply with guidelines
Make rounds to ensure nursing processes are being carried out effectively
