UAS RN Assessor

VillageCare is looking for self-motivated, passionate RN's for our Per Diem Nurse Assessor role. The Nurse Assessor conducts comprehensive assessments of all aspects of potential new members' condition - clinical, social and environmental to determine the level of services needed to appropriately keep member safe in the community.

Responsibilities will include:

• Utilize clinical skills to assess and document all aspects of the potential members long-term community based needs.
• Provide community-based training and education for frail population care.
• Conduct face to face UAS-NY assessments according to state guidelines, policies, procedures, and protocols.
• Gather clinical information from various sources to address all conditions.
• Communicate with consumers, families, providers, and other parties as needed to complete accurate comprehensive assessment.
• Explain VillageCareMax features and benefits, including explanation of the member's handbook.
• Consult with supervisor and others in overcoming barriers in meeting goals and objectives.

We would like to speak to those who have a current unrestricted NYS RN license along with 1+ year home care, hospice, or field based case management experience preferred. Proficiency in use of UAS-NY required. Prior MLTC/ Managed Medicaid/MAP/FIDA experience preferred. In home visits required. Bilingual skills required - Spanish, Korean, Russian, Cantonese, and Mandarin
