Assists in developing, implementing, and documenting individual support and behavior programs as appropriate. Maintains consistent records of individual attendance and progress. Provides a prescriptive and evaluative life enrichment program, implementing productive and meaningful activities. Develops and implements a schedule ensuring choice and active engagement during all program hours. Submits a monthly activity plan to meet program requirements and individual needs and interests by assigned due dates. Provides information to assist case management in developing and updating assessments, Individual Support Plans (ISP), and outcomes. Attends team meetings and contributes information on individual participation and progress, as requested. Responsible for following and providing support in accordance with the ISP and Behavior Support Plan (BSP), if applicable. Prepares, records, and submits accurate data to the designated team member(s) for all required reports according to due dates. Responsible for the oversight of assigned group areas. Creates a welcoming environment and delegates assignments to Direct Support Professionals (DSPs) and/or aides in the implementation of individualized programming and program site operations. Assesses and utilizes instructional techniques, materials, curricular resources, and professional consultation as necessary to enhance programming. Utilizes email and an electronic health record (EHR) to share, seek and record data, ensuring all necessary documentation for individuals (health, behavior, attendance, etc.) in the EHR is completed, up to date, and accurate. Maintains a clean, safe, and organized work environment, which includes being respectful of shared space and the assurance of inventory, needed supplies, and maintenance of all materials and equipment. Ensuring that materials and supplies are kept clean, sanitized, and in good working order. Assists with inventory, as assigned. Utilizes and maintains adaptive devices and equipment as stipulated in the individual’s ISP to assist the individual in the acquisition of functional skills to enhance performance in specific tasks. Assists with the individuals’ arrival to and dismissal from the program. Provides ongoing assistance and support in the areas of personal care and activities of daily living (including meal times, bathroom, etc.) as stipulated in the ISP. Participates in the implementation of programming in the most appropriate environment, including the residence. Responsible for maintaining familiarity with applicable licensing regulations and funding agency requirements. Dedicated to promoting and assisting with creative programming throughout Holland. Initiates ideas for holidays, special events and seeks out opportunities beneficial to the individuals. Remains in tune and knowledgeable of trends in the field. Meets and maintains all annual training and regulatory requirements. Attends and participates in staff meetings, and special events or assignments, as requested. Acts as a support to all Woods’ adult day programs, including participation in cross-training and shared programming, if appropriate. Displays professionalism and maintains effective communication and working relationships with all stakeholders, staff, and management. Performs all other duties as assigned to meet the needs and ensure the effectiveness and efficiency of the program. Must be a minimum of 18 years of age. Must be able to lift 50 pounds. Proof of a High School Diploma, GED, or College Degree is required. A minimum of one year’s experience in a habilitative setting-serving adults with developmental disabilities is required.
