Job Abstract

We are seeking to to fill a brand new role! We are on the lookout for a Nurse Practitioner that can help us in a Virtual capacity. This would have the occasional in-home visits. You would be supported by the community care LVN/LPN.

Job Responsibilities:

Care Planning
Problem-solving
Prescription writing

Required Skills:

Telehealth experience or being comfortable doing telehealth
Comfortable and philosophically aligned with the model
Background in family medicine

Typical Day
M-F
8-10 hrs/shifts
No on-call rotation
New Model: Virtual visits and occasionally will visit homes

Pay Details: $130,000.00 to $140,000.00 per year
