Job Abstract

Under the direction of the TLC Executive Director and in close coordination with the TLC College and Career Services Counselor, the Youth Outreach Coordinator is responsible for leading outreach efforts in the city of Pomona with systems-involved youth and coordinating individualized case planning with programming that prioritizes career-readiness, professionalism, timeliness, efficiency, equity, and addresses their general well-being. This position will require working closely with other youth-service providers across Pomona and forging collaborative relationships with the goal of collectively providing case management and stabilization services that help Pomona youth develop professional skills while meeting their comprehensive needs. The position will involve contributing to program development, conducting outreach activities, managing operational tasks, participating in team decision-making, developing data outcomes for youth, and supporting program outcomes for youth. This role plays a crucial part in ensuring the well-being and success of participants in the program.

PRINCIPAL DUTIES AND RESPONSIBILITIES
• Conduct developmentally appropriate intake and a needs assessment of incoming youth and connecting youth to needed resources
• Lead in the creation of personalized goal plans for systems impacted youth
• Develop youth-specific curriculum and activities for youth who have previous experience in child welfare systems, youth justice systems, and high levels of trauma
• Organize and facilitate youth activities onsite including leisure activities, celebrations, and opportunities for families to gather
• Provide ongoing advocacy-support for youth, including assisting with school enrollment, identifying work-based learning opportunities, directing them to social-emotional services, and ensuring families are supported in the transition
• Provide crisis intervention and emergency counseling for youth clients, Directing them to external support as needed
• Monitor youth progress on goal plans through regular check-in meetings, providing service support and other resources as necessary
• Assist with the development of data indicators to assess youth outcomes
• Communicate with parents, on an as-needed basis regarding parenting support, including assistance with parenting strategies
• Establish and maintain relationships with youth serving organizations, Participate in cross-training and team collaboration efforts
• Maintain the highest levels of accuracy in record-keeping and reporting, ensuring that all client files and documentation are complete and up to date
• Other duties as assigned

SKILLS, KNOWLEDGE AND ABILITIES
• Bachelor’s degree in counseling, social work, psychology, child development or related field; or equivalent combination of education and related experience.
• Two years experience working with children, youth, adolescents, or families from underserved communities.
• One-Two years experience working with at-promise youth (Systems-Impacted youth).
• 6 months experience in the field of family violence intervention, client services, and/or crisis intervention.
• One year experience providing crisis intervention, group activities, advocacy and/or case management.
• One year experience with Los Angeles foster care systems and Los Angeles County youth probation system.
• 6 months experience in the field of family violence intervention, client services, and/or crisis intervention.
• Bilingual oral and written fluency in Spanish.
• Strong interpersonal and communication skills, including conflict resolution skills.
• Strong leadership, facilitation, and public-speaking skills for small groups and large settings.
• Must demonstrate respect for cultural diversity in all its forms, including ethnicity, linguistic, abilities, genders, and religious backgrounds.
• Computer proficiency with Microsoft Office Applications (Word, Excel, Outlook, Power point, Access).
• Organized and detail oriented with the ability to effectively multi-task, prioritize, and meet deadlines.
• Ability to work as part of a team and manage individual responsibilities and multi-tasking.
• Ability to maintain a proactive approach and execute all duties in their entirety.
• Proactively adapt to always changing requirements and duties.
