A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates
Hybrid Case Worker Responsibilities
Respond, either verbally or in writing, to select inquiries from intra and inter-departmental units, outside entities and family/applicants.
Reviews and recommends modifications to program manuals and materials based on policy revisions. Develop and update forms and files.
Coordinates the collection of Data to prepare reports.
May be Involved in incident reviews, closing incidents, reports and investigations.
May be involved in fair hearing preparation.
Participates in trainings, meetings and conferences on behalf of the Bureau as assigned.
Assist in tracking information, reviewing reports, and preparing reports for the supervisor, and others when needed.
Review previously initiated protective services cases and perform investigative activities to complete the investigation. This will include making telephone calls to collateral contacts such as family members, alleged perpetrators, physicians, banks, neighbors, friends, eyewitnesses, care providers, and other individuals or entities. Work must be completed in accordance with protective services legislation, regulations, and operating policy and procedures of the OAPSA. Comply with legal and statutory time frames.
Identify any unmet needs or potential risks to the older adult.
If any unmet needs are identified or any potential risks identified, refer the case back to the AAA for services (preparation and implementation of a PS service plan).
If no unmet needs or potential risks are identified, close the case record in accordance with the PS database manual.
Utilize the professional services of agency-based medical professionals to assist with case-related assessments when necessary.
Document all case-related actions and results (i.e. interviews with collateral contacts, review of medical or financial records, obtainment of a Power of Attorney document, etc.) and enter all required information into the PA Department of Aging’s standardized documentation system.
Hybrid Case Worker Requirements
Bachelor Degree in human services, social work, psychology, sociology, or similar
2 years either in research or providing social services required
1 year of direct aging casework preferred
