What will you be doing in this role:
The Clinical Social Worker LCSW provide a full range of social work services in collaboration with other CSMC staff to meet the psychosocial needs of the Heart Transplant patients/families/significant others/caregivers. Provides social work services and care coordination topatients and their families with psychosocial needs. Active participation in Department's continuing education. Involved inprogram planning. Will be integral part of transplant multidisciplinary team.
Psychosocial evaluation -- identification of problems affecting optimal patient care and the development of a treatment plan.
Crisis intervention.
Individual and family counseling.
Discharge planning and case coordination.
Interdisciplinary discussion of psychosocial implications of illness including consultation regarding treatments and resources designed to solve the problems of the patients.
Develops and maintains productive relationships with community health, welfare, and social agencies.
Initiates program planning and development in assigned services/programs.
Participates in development and leadership with Department's Continuing Education.
Performs other related duties as directed.
Qualifications
Experience Requirements:
Three (3) plus years of experience of Acute care hospital setting, preferably in transplant.
Hospital and/or medical clinic experience preferred that provides care for several patient populations.
Educational Requirements:
MSW – Master’s Degree of an accredited School of Social Work.
LCSW – Licensed Clinical Social Worker.
