Job Abstract

Reporting to the Manager of the Heart Transplant and Mechanical Assist Device Program. This position, in collaboration with the Medical and Surgical Directors, cardiologists and other health care providers, will be responsible for the oversight of the transplant wait list and coordination of the patient evaluation process being considered for organ transplantation. Acts as the clinical liaison with various departments involved in the care of transplant patients.  Provides education to patients and families.  Ensures adherence to protocols and program certification requirements, compliance, quality assurance and performance improvement. Coordinates a comprehensive plan of care for a designated group of patients. Assesses and identifies patient care issues and facilitates progress toward expected outcomes. 

Required

Associate's Degree; Bachelors Degree in Nursing is preferred

New York State Nursing License required

Three years of clinical experience in a direct patient care setting

Clinical experience in a highly specialized complex cardiac care program preferred

Transplant experience preferred 

Computer literate

Effective English communication skills, both written and oral
