The Team Lily Perinatal Navigator involves providing non- clinical support, guidance and assistance for patients and families affected by substance use, unstable housing and homelessness and significant mental illness as they navigate complex healthcare environments. Primary goals are to increase this high-risk patient populations’ engagement with the healthcare system, satisfaction, and prevention of far too frequent outcomes such as falling out of care, traumatic experiences with the healthcare system, and missed opportunities for prevention and treatment, specifically as families move from prenatal to birth to postpartum. Acts as a communication liaison to understand the patient's non-clinical individual needs, desires, and concerns, and relays this information to the clinical team to facilitate patient-centered care. Navigates the patient and family to a broad range of clinical and non-clinical services, amenities, and information to (1) promote attention to patients’ stated needs and care engagement, (2) support engagement in substance use treatment, (3) improve pregnancy and postpartum outcomes, and (4) increase satisfaction with the patient care experience.The Team Lily Perinatal Navigator serves as a non-clinical resource and liaison for pregnant patients and families with young children affected by substance use, housing instability, and / or mental illness. Assists with navigating all aspects of the healthcare system, with focus on increasing communication from prenatal to postpartum services to enhance patient safety and in turn helping to prevent poor outcomes ranging from relapse and overdose, to congenital infections, to maternal and neonatal morbidity and mortality. Uses knowledge of the clinical team to resolve issues and identify ways to increase patient satisfaction and engagement. Collaborates with department managers to meet patient expectations, achieve quality outcomes and build a patient focused culture.The Team Lily Perinatal Navigator is a critical role in support of the clinical and logistical needs of patients during pregnancy to enhance coordination of services and patient safety and as they transition from prenatal to birth center to postpartum / pediatric care. In caring for marginalized families during this important time in their lives, particularly those affected by substance use and homelessness, we employ a multidisciplinary team- based care model. This position follows national trends in case quality improvement for hospitalized patients, focusing resources and support on collaborative planning and communication between patient, inpatient, and outpatient medical teams and community-based organizations. The Perinatal Navigator will be an integral part of Team Lily, attending weekly rounds and working closely with physicians, nurses, pharmacists, social workers, and other outreach workers. Specific tasks include outreach efforts and care coordination for the most at-risk pregnant individuals, including outreach to individuals who are currently unsheltered; meeting with patients during hospitalization; acting as a patient-focused liaison between the inpatient service, ambulatory care (primary and specialty care) and community-based organizations; and making post-discharge phone calls to patients.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $55,200 - $95,900 (Annual Rate).
